APPLICATION FOR CORRECTION OF PROPERTY TAX ASSESSMENT DATE: MARCH 1, 2024
I hereby authorize P.T.R.C., Inc. to process my Application For Correction/Refund Application for filing in calendar year 2024 immediately, as my exclusive agent, with the Nassau County Assessment Review Commission and before any county and/or village/city Assessment Review Commission/Small Claims Assessment Review of the Supreme Court, State of New York, for correction of the assessment appearing on the tentative roll published in Jan. 2024 and all corrections for the 2025/26 tax year. I authorize ARC, Nassau County Department of Assessment and/or village/city to communicate directly with P.T.R.C., Inc. in all matters relating to this application.
1- Eligibility: 1. A person named in the records of the Nassau County Clerk as a homeowner; or 2. That person's authorized agent; or 3. A person who has contracted to buy a home; or 4. The estate of a deceased homeowner, is eligible under law to receive a tax assessment reduction and a property tax refund. 5. The property is a one, two or three family home used exclusively for residential purposes. If you are not in any of these categories, you will not be able to receive a property tax refund and you should not sign this agreement. If you are in one of these categories, you may sign this agreement; and 6. Complaints regarding any services rendered or not rendered under this contract may be addressed to the Nassau County Office of Consumer Affairs.
2- Service to Be Performed: Note that you are not required by law to use a tax reduction service in order to file for and/or receive a tax assessment reduction. By signing this agreement, however, you are directing P.T.R.C., Inc. (not affiliated with any municipality) to prepare and file your First Level Complaint for review by the Assessment Review Commission, to represent you at any proceeding which includes the preparation and filing of the Petitions, the preparations and presentation of a Market Analysis and to physically appear on your behalf at settlement conferences and all court hearings as necessary. P.T.R.C., Inc. will prepare, and file forms required by any municipality for issuance of a possible refund check, minus any fees due and the balance forwarded to you within 30 days from our receipt. If there is a duplicate filing, P.T.R.C., Inc. may withdraw its filing.
3- Notification Of Tax Reduction: P.T.R.C, Inc. is required by law to make reasonable efforts to communicate the terms of any settlement offer made in the course of a tax assessment review proceeding, other than a hearing or trial although I hereby give P.T.R.C., Inc. full authority to settle my case and negotiate any municipality refund checks obtained and deduct its fee therefrom.
4- Fee For Service: P.T.R.C., Inc. DOES NOT CHARGE A FEE IF THERE IS NO REDUCTION. If successful, I agree to pay a fee equal to 50% of the Tax Reductions payable within 60 days of the date of P.T.R.C., Inc.'s invoice containing official proof of reduction. A $50 late charge will apply to all payments 60 days past due, should my case(s) require an appeal a NYS court filing fee of $30 may apply. If I do not pay within 60 days, I shall pay (i) 1% interest per month on the amount due from the date of the first invoice; and (ii) all costs of collection including attorney's fee of 33% of any amount due including interest. I also agree that P.T.R.C., Inc. may submit its claim to an arbitrator or arbitration company selected by P.T.R.C., Inc. (P.T.R.C., Inc. shall advance any arbitration fees, but the arbitrator shall award such fees to the prevailing party) or bring suit against me in the courts located in Nassau County; except if P.T.R.C., Inc. demands arbitration, Nassau County courts shall have exclusive jurisdiction of any dispute/action relating to this agreement; and service of a demand for arbitration or summons and complaint upon me by mail at the address listed in this agreement shall be sufficient service and notice thereof. Sale of my home does not void this contract, and I will remain responsible for any fees incurred unless I have my buyer assume the responsibility of this contract in writing with P.T.R.C., Inc.
5- Cancellation: At any time within three (3) days after entering into this contract, I have the complete right to cancel this agreement in writing to P.T.R.C., Inc.

APPLICATION FOR CORRECTION OF PROPERTY TAX ASSESSMENT DATE: MAY 21, 2024
I hereby direct P.T.R.C., Inc. to process my Property Tax Reduction/Refund petition for filing in calendar years 2024/25 as my sole agent pursuant to my rights guaranteed by New York State Real Property Tax Law.
1- Eligibility: 1. A person named in the records of the Suffolk County Clerk as a homeowner; or 2. That persons authorized agent; or 3. A person who has contracted to buy a home; or 4. The estate of a deceased homeowner is eligible under law to receive a tax assessment reduction and a property tax refund. If you are not in any of these categories, you will not be able to receive a property tax refund and you should not sign this agreement. If you are in one of these categories, you may sign this agreement. 5. If you have NOT received a tax reduction in 2023.
2- Service to Be Performed: Note that you are not required by law to use a tax reduction service in order to file for and/or receive a tax assessment reduction. By signing this agreement, however, you are directing P.T.R.C., Inc., (not affiliated with any municipality) to prepare and file your First Level Complaint for review by the Board of Assessment Review, to represent you at any Appeal proceeding which includes the preparation and filing of the Appeal Petition and to physically appear on your behalf at your Appeal Hearing with the Court appointed hearing Officer and the Assessor’s representative. P.T.R.C., Inc. will negotiate any refund check from the Suffolk County Treasurer, minus any fees due and the balance forwarded to you within 30 days from our receipt.
3- Notification Of Tax Reduction: P.T.R.C., Inc. will make every reasonable effort to communicate the terms of any offer of settlement made by Suffolk County in the course of the tax assessment review proceedings as required by law. I fully authorize P.T.R.C., Inc., to negotiate a settlement for me.
4- Fee For Service: P.T.R.C., Inc. DOES NOT CHARGE A FEE IF THERE IS NO REDUCTION. I agree to pay a fee equal to 50% of the reduction prior to exemptions on my property taxes, payable within 30 days of the notification of the tax reduction, and to pay such fee in the event I sell or move out of the subject property. If the assessment is reduced the Actual tax savings = the reduced assessment multiplied by the tax rate prior to exemptions. If I default on said payment, I agree to pay a fifty-dollar late fee after 30 days and, if needed, reasonable collection/attorney’s fees incurred by P.T.R.C., Inc. and 1% interest per month on the amount due from the date of the invoice. I also agree that the courts located in Nassau County shall have exclusive jurisdiction of any dispute or action relating to this agreement, and that service upon me by mail at the address listed in this agreement shall be sufficient notice of any such dispute or action. A $30 court filing fee may apply. I also agree to pay a $75 market analysis fee or supply a certified appraisal (issued within 1 year) with my application. No other fees or charges. At any time, within three (3) days after entering into this contract, I have the complete right to cancel this agreement by written notice to P.T.R.C., Inc.
